Download and save the form
Click the download button above and save the PDF to your computer or phone before you start typing.

Open it in a PDF app
Use the free Adobe Acrobat Reader (computer, iPhone or Android) or Preview on a Mac. Some web browsers will not keep your answers when you save, so avoid filling it in inside a browser tab.

Fill in the details
Click into each box and type. Fields marked with a * are required. Choose Yes or No for the two questions, and enter each wall's width and height in feet. If your project has more than four walls, list the extra measurements in the comments box.

Gather your photos and plans
Take a photo of the wallpaper label and of each wall. If you have architectural drawings or plans, have those ready too. Tick “Attached” on the form for each one you will send.

Save and email it to us
Save the completed PDF, then email it with your photos and plans attached to jfuglestveit@gmail.com.
